You must report the loss or theft of your passport as soon as possible.
Please ensure your passport is definitely lost before you contact us as your passport will be cancelled immediately and permanently.

You will need to follow steps 2 to 6 at the below link. Once you have followed those steps, return to this page for further instructions.
You will need to complete a B11 General Declaration By Passport Applicant Form. Please CLICK HERE to see the list of supplementary forms which you can download.
Print the B11 form and complete it by hand. Digital signatures are not accepted.
Please complete your personal details at the top of the form. Then write a detailed statement explaining what happened to the passport and the steps you have taken to try to find it. Then complete the signature declaration at the bottom.
For child applications, the lodging parent must complete the form on behalf of the child applicant.
Complete one B11 form for each applicant.
You must submit your application in person. There is no option to submit your application by post. No one else can submit your application on your behalf.
Children aged 16 and 17 years must attend the appointment along with one parent.
Children aged 15 years and younger do not need to attend the appointment.
You must book an appointment. There is no walk-in service.
Do not bring any family members or friends with you.
You must clear through security checks to enter our building. Please arrive at least 10 minutes before your appointment time.

You must pay the full fee when replacing a lost or stolen passport.
The fee for an adult passport is AUD$596.
The fee for a child aged 16/17 years is AUD$502.
The fee for a child aged 15 years and younger is AUD$298.
If you are aged 75 years or older, you can choose to apply for either the 10-year passport (AUD$596) or a 5-year passport for a reduced fee of AUD$392.
Payment is processed at the end of your appointment once your application has been checked.
Payment is processed by credit or debit card and we accept VISA, Mastercard, and AMEX. Contactless is not available.
We take payment in Australian Dollars only. You may be charged a fee by your bank if you use a UK card.
Passport applications are currently taking up to six weeks from the day we process payment.
Your new passport will be posted to the UK residential address that you list on your application form. We send passports by Royal Mail Special Delivery, which is a registered and signed-for service.
There is no priority service or fast track option available. If the applicant has urgent travel within three weeks, please CLICK HERE.
You should notify us if you find your passport.
If you have already reported your passport as lost or stolen then you cannot use it. You will need to apply for a new passport.
If you find your passport and try to use it, it may be confiscated by border officials.
If you show us the found passport, we may not count the loss or theft against your passport record. You will also need to write another B11 General Declaration By Passport Applicant Form with a statement explaining how you found it.
